Step 3: Mail all the samples back to the lab.

After all the tested parties have been collected, put their sample collection envelopes back in the Beta Self-Collection™ Kit.

Next, complete and sign the Client Information Form. This form is where you indicate how you would like to pay for the test and receive your results.

Place the kit and the Client Information Form inside the postage-paid Business Reply envelope. Seal the envelope securely, and drop it in the mail.
